It is especially important for us to have the correct VOICE # or the VOICE Coordinator will not be able to update your Tech Section transcript. If you have special needs (ex. State of Illinois licensure credits, please note them in the box with your #).

Please read all answers before you make your selection- only ONE answer is correct. Please select the box next to the answer of your choice. 

 When your test is completed, You can either Press the "Submit" button at the bottom of the page, sending us your answers via email-- OR -- Print the completed form with your answers and send it to the address listed above. 

• If you choose to use email, you must submit your payment via the paypal links from the appropriately selected tutorial. 

• If you wish to pay by check, you must print this completed form and send a check for $20.00 to Consultants in Nuclear Medicine (address above). The charge is $30.00 for tutorials which award 2 CE credits.
  

If your score on the test is 80% or greater, then CNM will send you a certificate verifying your participation in this VOICE credit-approved program and will request that the VOICE Coordinator credit your Tech Section account for 1.0 or 2.0 CEU. If you scored less than 80%, we will notify you and you can take the test again. There is no charge for the first retake.

If you are a Nuclear Medicine Student, You can take the quiz and use the tutorials at no charge. You will not receive any CE credit. Please "note" in the box on the form that you are a student.
